Results Delivery Platform for a Dutch Medical Laboratory
Designed and built a clickable lab console for an ISO 15189 accredited laboratory in Zuid-Holland: follow every order and sample from collection to storage, read validated results with flags, EUCAST susceptibility and release state, and phone critical values against a deadline with a logged call chain and a read-back check.
Most of our work is under NDA. Per agreement, we can share the work but not the company details - all names and identifying information shown here have been changed.
Dashboard, orders and samples with the sample tracker, order detail with 5 tabs and its cumulative history panel, the critical values queue and the call page.
Each critical value counts down from 30 minutes in office hours or 60 out of hours. Over-deadline calls turn red and move to the top, and a call only closes on a matching read-back from an authorised person.
HIV, PSA and pregnancy results are held for the doctor's release for at most 14 days. The order's Release tab shows the rule behind every result: released, delayed, held or withheld.
26 patients, 11 requesters at 9 GP, midwife and hospital practices, 22 catalogue tests with LOINC codes and 12 critical value calls in every status.
Duinhof delivered results four ways: Edifact into GP systems, HL7 to hospitals, faxes to midwives and phone calls for anything urgent. Failed messages went unnoticed until a GP called, critical values were phoned from a paper list and logged in a notebook, patients had to ask their GP for their own results, and the quality manager spent days assembling ISO 15189 audit evidence.
Baken sits on top of the lab information system and covers the path after a doctor decides to test: ordering, sample tracking, validated results with reference ranges and flags, critical value calls with read-back, delivery to the doctor's own system, and patient release under the doctor's policy, with every view of patient data logged.
A Dutch lab serving around 420 GP practices, two hospitals, midwives and clinics has to get each result to the right doctor in the right format, phone the dangerous ones, release the rest to patients on the doctor's terms, and prove all of it to auditors and privacy officers.
Results beyond Duinhof's critical limits must be phoned to a responsible clinician who reads the value back. With a paper list and a notebook, the lab could not show who was called, who answered, what they read back or when.
Edifact MEDLAB via ZorgMail, HL7 and FHIR to hospital systems, secure mail and phone each failed in their own way. Nobody saw a failed message until a GP asked where a potassium result was.
Patients have a right to their record under the WGBO and to electronic access under the Wabvpz, but when a result appears is the requesting doctor's decision. Sensitive tests such as HIV, tumour markers and pregnancy tests are held until the doctor releases them or a set delay passes.
Lab results are special category data under the GDPR. NEN 7513 requires a log of who opened which patient's data, the BSN must be verified against SBV-Z and never shown in full outside the lab, and patients sign in with DigiD at level Substantieel.
Mapped 10 roles from analyst and duty officer to requester and patient, 37 screens across the lab console, requester portal and patient portal, and the data model for orders, samples, results, cultures, release states, critical calls, amendments, messages and the NEN 7513 access log. Wrote down the rules the design follows: GDPR and the UAVG, WGBO, Wabvpz, Wegiz, Wbsn-z, NEN 7510, 7512 and 7513, MedMij, ISO 15189, LOINC and NHG coding and EUCAST breakpoints.
Built 5 key screens of the lab console on a production dashboard kit: the dashboard with KPIs, hourly sample flow, turnaround by priority and a My work list per role, orders and samples with status filters and the sample tracker, order detail with results, release, delivery, critical values and activity tabs, the critical values queue with live countdowns, and the call page with attempts, contact recording and escalation. All actions run on local state.
Integration with the LIS (such as GLIMS), ZorgDomein ordering, ZorgMail Edifact delivery, HL7 and FHIR feeds to hospital systems, DigiD, SBV-Z, UZI card login, the BIG register and MedMij for PGO sharing. A tamper-evident access log, the release queue, amendments with two-person approval, the message monitor, reports, settings, and the requester and patient portals.
Every critical value shows the test, value, limit, requester and a countdown to its deadline. Over-deadline calls are red and on top, and a duty officer takes a call with one click, so each call has one owner.
The call page lists who to phone in order, from the requester's direct line to the practice, the on-call number, the out-of-hours GP post and the hospital on call, each with click-to-call. Previous values sit beside the result, so a known chronic abnormality is easy to spot.
Recording a contact asks for the person, their role, whether Duinhof policy allows them to receive the value and the value as they read it back. A read-back that does not match is refused, and the deadline banner names the next number to try.
The BSN is masked with a reveal button that is logged, its SBV-Z check is shown on the patient card, and each result carries its release state and the rule that set it, so staff can answer a doctor or a patient without guessing.

Built a multi-department clinic operations console with appointment management, department load tracking, staff performance, resource utilisation and an AI assistant for staff queries - shipped from clickable AI prototype to production in 14 weeks.

Designed and built a clickable booking and intake platform for a group of GP, physiotherapy and psychology clinics in regional Victoria and southern NSW: a day calendar by practitioner or room, pre-visit forms reviewed before the visit, and a clinician-only queue that puts every risk answer in front of a clinician against a response target.
