Nadi - Video Consult Platform with Guideline-Aware Prescribing

Teleconsultation Platform for an Indian Telemedicine Provider

Designed and built a clickable ops console for an Indian telemedicine provider's video consult platform: a live queue across four specialties, every consult with its guideline evidence and signed e-prescription, refunds within role limits, and prescribing flags raised when a doctor tries a medicine the Telemedicine Practice Guidelines do not allow.

Impact

5 Screens
Clickable end to end

Ops dashboard, live queue monitor, consults list, consult detail and prescribing flags. Every one of the 30 demo consults opens its own detail page.

4 Lists
Medicine rules on every Rx

List O in any mode, List A only in a video first consult or a follow-up refill, List B only as an add-on in a follow-up, and prohibited medicines never. Blocked additions show on the consult with the rule that stopped them.

5 Flag Types
Raised automatically

Blocked medicine attempts, antibiotics above the specialty median, amendments after signing, prescriptions without an identity check and consults under 3 minutes with a prescription. Each can be dismissed with a note, sent to audit or followed up with the doctor.

30 Consults
Realistic demo data

16 doctors with council registrations, 24 patient profiles across 18 family accounts, a 21-medicine formulary slice, 14 prescribing flags and 10 more patients waiting in the live queue.

The Problem

Tulsivan Health grew from a phone-in helpline: an agent booked patients into a doctor's WhatsApp video call, payment was a UPI screenshot and the prescription was a photo of a letterhead. Nobody could prove which doctor saw which patient, whether identity was checked, or whether a medicine given on an audio call was allowed on one. Calls dropped on 2G with no fallback, refunds were settled from a spreadsheet, and around one consult in six ended in a complaint or a refund request.

Our Solution

Nadi takes a patient from a phone OTP sign-in to a registered doctor by video, audio or chat, and a digitally signed e-prescription. The prescription writer blocks medicines the guidelines do not allow in that consult, the call steps down from video to audio, chat or a phone bridge when the network is weak, and the ops console shows the evidence for every rule on each consult record.

The Challenge: Teleconsultation Under the Guidelines

In India the Telemedicine Practice Guidelines decide who may consult, how identity and consent are recorded, and which medicines a doctor may prescribe, and the answer changes with each consult: first or follow-up, video or audio, adult or child.

Medicines Depend on Mode and Consult Type

List A medicines need a video first consult or a follow-up refill, List B can only be added in a follow-up for a condition already under treatment, and Schedule X and NDPS drugs can never be prescribed remotely. A doctor cannot hold that matrix in their head mid-call.

Proof of Identity, Consent and Registration

The doctor must confirm the patient's identity, a minor must be seen with an adult present, consent must be recorded when a doctor or caregiver starts the consult, and every prescription must carry a verified State Medical Council registration number.

Consults on 2G and Patchy 4G

Patients join from Pune to Patna on whatever network they have. A dropped video call meant a lost consult, a refund by hand and a complaint, with no record of what happened on the line.

Psychiatry and Health Data Privacy

Health data is processed on consent under the DPDP Act 2023, and psychiatry consults carry a stronger confidentiality expectation under the Mental Healthcare Act 2017. Support and finance staff need the who, when and what was paid, but never the notes.

Our Approach: From Telemedicine Rules to a Clickable Console

01

Spec and Rules Research

Mapped 9 roles and 55 screens across the patient app, the doctor app and the ops console, and the data model for profiles, consents, doctors, consults, call sessions, the versioned formulary, signed prescriptions, refunds, payouts and quality audits. Wrote down the rules the design follows: the Telemedicine Practice Guidelines 2020, the Drugs and Cosmetics Act and Rules, the NDPS Act, eSign under the IT Act 2000, ABDM and ABHA linking, the DPDP Act 2023, the Mental Healthcare Act 2017 and TRAI DLT templates for SMS.

02

Clickable Demo on a Production UI Kit

Built the ops console's 5 key screens on a production dashboard kit, signed in as the clinical quality lead: the dashboard with consults per hour, waits per specialty and call quality step-downs, the live queue monitor with reassign, call back and cancel with refund, the consults list with filters, the consult detail with prescription, guideline evidence, timeline and a refund panel, and the prescribing flags queue. Actions update the screen as you click.

03

What a Production Build Adds

The patient app and the doctor app with the consult room and prescription writer, video through a provider such as 100ms, Agora or Daily, OTP and voice fallback through MSG91 or Exotel, UPI payments and payouts through Razorpay or Cashfree, Aadhaar eSign through eMudhra or NSDL, the ABDM gateway, council register lookups and doctor KYC. Staff auth by role, an append-only audit log, and the remaining ops screens for onboarding, audits, payouts, support, reports and settings.

Why It Works: Every Consult Carries Its Evidence

Guideline Evidence on the Record

Each consult lists its teleconsultation consent, identity check, adult present for a minor, consult type, the highest mode actually used and the medicine list checks, each passed or failed, with the formulary and policy versions in force when it started.

Blocked Medicines Explain Themselves

A refused addition shows the medicine, its list and the rule, such as "List A needs a video first consult", right on the prescription. Every attempt feeds the prescribing flags queue, where flags are also counted by doctor next to their audit score and antibiotic rate.

Calls That Step Down, Not Drop

The record shows each step from video to audio, chat or a phone bridge with both sides' network. The live monitor marks patients returning after a drop, and the dashboard shows how many video consults stepped down and to what.

Restricted Records and Refund Limits

Psychiatry records show a notice that they are hidden from support and finance and that the view is logged. A refund above the user's own limit goes to Finance for approval instead of being paid straight away.
