Lysning - Type 2 Diabetes Programme with a Coach Console and GP Escalations

Diabetes Coaching Programme for Danish Municipalities

Designed and built a clickable coach console for a Danish type 2 diabetes programme that municipalities buy for their residents: a caseload sorted by risk, rule-based flags with a service standard, a full participant record, and letters to GPs that go through clinical review and MedCom delivery.

Impact

5 Screens
Clickable end to end

Caseload, risk flags, escalations, escalation detail and the participant record with 7 tabs. Each of the 24 demo participants and 12 escalations opens its own page.

7 Flag Types
Raised by clinical rules

Hypo, high reading, rising average, missed logging, medication gaps, disengaged and device quiet. Each comes with a suggested next step, and high flags turn amber after 1 working day and red after 2.

7 Statuses
GP letter workflow

Draft, clinical review, sent, delivered, GP replied, closed and failed. Letters marked within 3 working days, and any letter from a coach in their first 3 months, go to a clinical lead before sending.

24 Participants
Realistic demo data

21 risk flags, 12 GP escalations, 5 municipalities, 6 GP clinics and 4 staff users, in Danish formats: mmol/L, HbA1c in mmol/mol and DD.MM.YYYY dates.

The Problem

Fjordlys ran its diabetes programme on a generic coaching app, a spreadsheet per municipality and phone calls. Glucose readings arrived as screenshots, coaches could not see who had stopped logging until the next call, hypoglycaemia reports sat in chat threads with no follow-up trail, and letters to GPs were written in Word and faxed or posted. Two municipalities threatened not to renew because they could not see outcomes for their own residents.

Our Solution

Lysning gives each participant a 12-month place with device data, logging, short lessons and weekly coaching from an authorised nurse or dietitian. The coach console shows who needs attention today, raises flags from approved rules, keeps every note as a patient record, and sends structured letters to the GP through MedCom when something needs a medical decision.

The Challenge: Coaching Without Crossing Into Treatment

Lysning is delivered on a municipality's behalf by authorised health professionals, for people whose medicine is managed by their own GP. Every screen has to support daily coaching while staying inside Danish health, data protection and medical device rules.

Risk Hidden Between Calls

A coach works a caseload of about 120 participants. Lows below 3.9 mmol/L, a rising 14-day average or two weeks without logging were only noticed at the next scheduled call, if at all.

Coaching, Not a Medical Device

Software that suggests patient-specific doses is generally a medical device under EU MDR Rule 11. Lysning shows readings and trends, never suggests insulin or tablet doses, and routes anything that needs a treatment change to the GP.

Notes That Are Patient Records

Coaches are authorised health professionals, so their notes fall under the Danish record-keeping rules and are kept for at least 10 years in most cases. Notes can never be deleted, only corrected with a visible trail.

Three Parties, Different Data Rights

The GP needs a structured letter, the municipality only aggregated, pseudonymised outcomes, and the participant chooses when a summary goes to the GP. Health data is special category data under GDPR, so every view has to respect who may see what.

Our Approach: From Care Rules to a Clickable Console

01

Spec and Rules Research

Mapped 7 roles across four audiences (participant app, coach console, programme admin and municipality portal), 48 screens and the data model for referrals, contracts, participants, consents, readings, hypo events, flag rules, notes, escalations and municipal reports. Wrote down the rules the design follows: the Danish Health Act, record-keeping duties, GDPR and the Danish Data Protection Act, the EU MDR device boundary, MitID, MedCom messaging, mmol/L and IFCC units, and WCAG 2.1 AA.

02

Clickable Demo on a Production UI Kit

Built the coach console's 5 key screens on a production dashboard kit: the caseload with KPIs, needs attention today, check-ins due and a filterable caseload table, the participant record with glucose charts, a 12-week logging heatmap, hypo list, notes with struck-through corrections and an access log, the risk flag queue with resolve and snooze, and the escalation list and letter detail. The full letter workflow, from draft through clinical review, delivery receipt, GP reply and a failed delivery, runs on local state.

03

What a Production Build Adds

MitID login through a certified broker and MitID Erhverv or Entra ID for staff, Apple HealthKit, Health Connect, Dexcom and LibreView data, a MedCom-certified integration partner for GP letters, Digital Post, push and SMS reminders and EU-hosted video for group sessions. A tamper-evident audit log, versioned consents and flag rules, and the remaining screens: the participant app, programme admin and the municipality portal.

Why It Works: The Coach Sees Risk First

Today's List Sorted by Risk

The caseload opens on participants sorted by their highest open flag, then by the oldest unanswered message, with last log and last contact on every row. A coach knows who to call before reading a single thread.

Flags Explain Their Trigger

Each flag states its trigger in plain words, such as 3 readings below 3.9 in 7 days, with a small chart, the suggested next step and its age against the service standard. Resolving one needs an outcome and writes a note to the journal.

The System Never Writes Treatment Advice

A GP letter is built from the coach's observation, the relevant data, what the participant has been told and a question for the GP. Lysning attaches the evidence but never adds a recommendation of its own.

Records That Keep Their History

Corrected notes show the original struck through with who changed it and why, target range changes need a reason within set limits, and the Data & access tab lists everyone who opened the record.
